This notice applies to all information collected or submitted on the Storms & Swiderski Orthodontics website and social media sites. On the Patient Login page, you have the ability to access your personal orthodontic account by way of a personal ID and Password. Once your account has been accessed, you may also make requests (submit a change of dentists, supply us with new insurance information, etc.). You are also able to make online payments via OrthoBanc on our site. The types of information collected would be: name, address, e-mail address, phone number, credit/debit card information, bank routing/account number, etc.
This information is not shared with outside parties other than OrthoBanc, a secure site who processes all our online payments. To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, we have put in place appropriate physical, electronic, and managerial procedures to safeguard and secure the information collected online.
You may access all your personally identifiable information that we collect online and maintain by contacting our office directly. You may also request correction of factual errors in your personally identifiable information by sending us a request that credibly shows error. To further protect your privacy and security, we will also take reasonable steps to verify your identity before granting access or making corrections.
